Men’s hormone therapy

Testosterone, evaluated and replaced at the level your body actually runs on.

For men whose energy, recovery, drive, and focus aren’t where they used to be. Full hormone panel, physiologic TRT, estrogen management and intratesticular preservation built in. The goal is your optimal range, not a clinic’s normal range.

The symptom stack that brings most men in

Testosterone declines about one to two percent per year starting around 30. For some men this is barely noticeable. For others it lines up with a specific drop in energy, drive, recovery, and clarity that gets attributed to age, work, or stress. Men come in when several of the following overlap:

  • Energy that doesn’t hold across the day, regardless of sleep.
  • Recovery from training that takes days where it used to take hours.
  • Strength and muscle that have gone backwards without a clear cause.
  • Visceral weight gain that diet and exercise aren’t moving.
  • Libido and morning erections that aren’t what they were.
  • Mood drift: shorter fuse, lower motivation, flatter affect.
  • Brain fog, sluggish focus, word-finding lapses.
  • Sleep that no longer feels restorative even when the hours are there.

Any one of these could be other things: thyroid, cortisol, sleep apnea, nutrient depletion. That’s why we don’t start TRT on a single low testosterone reading. We run the panel first.

What we actually evaluate

Many PCPs check a single total testosterone, see a number inside the reference range, and stop. Reference ranges in the US are built off populations that include men who already feel terrible. “Inside normal” isn’t the same as “at your level.” Our men’s panel covers:

  • Total and free testosterone: the circulating hormone and the fraction actually available to bind receptors.
  • Estradiol. Men need estrogen, but elevated estradiol drives fatigue, water retention, gyno risk, and mood changes.
  • SHBG. The protein that binds testosterone, often the reason a total reading looks fine while free testosterone is low.
  • DHEA-S: adrenal precursor for testosterone and a marker of resilience.
  • Full thyroid panel: TSH, free T3, free T4, reverse T3, antibodies.
  • Cortisol: chronic stress suppresses testosterone directly.
  • IGF-1: the practical readout of growth hormone activity, relevant for recovery and body composition.
  • Insulin and metabolic markers: insulin resistance suppresses testosterone and accelerates the spiral.
  • PSA, CBC, CMP, lipid panel: prostate baseline and safety markers, required before and during TRT.

How we treat, in plain language

Testosterone replacement therapy (TRT)

When the labs and the symptoms line up, TRT is the central intervention. Delivery is typically weekly injections or pellets. We pair it with two things almost always: an aromatase inhibitor to control the estradiol that the body converts testosterone into, and a medication to preserve intratesticular testosterone production while you’re on therapy. We dose to the level where you feel right, not just into the wide reference band. We monitor on a defined cadence and adjust.

Thyroid optimization

Thyroid dysfunction is one of the most underdiagnosed conditions in men. A single TSH reading misses subclinical hypothyroid patterns that quietly drag energy, recovery, and body composition. We work with the full thyroid panel and treat to function, not to disease absence. For many men, thyroid optimization is the missing piece that makes TRT work the way they expected.

Cortisol and adrenal support

Chronically elevated cortisol suppresses testosterone directly, promotes visceral fat, breaks down muscle tissue, and disrupts sleep. If cortisol is out of pattern, optimizing testosterone alone won’t get you where you want to be. We evaluate cortisol patterns and address the root causes.

DHEA and growth hormone support

DHEA supports testosterone production, immune function, and overall vitality, and declines naturally with age. Growth hormone is rarely directly replaced in our protocols, but we evaluate IGF-1 and, when appropriate, use targeted peptide therapy and lifestyle interventions to support recovery, sleep, and body composition.

Estrogen management

Men need estrogen for bone, libido, and cardiovascular function. Too much causes the problems mentioned above; too little causes its own. On TRT, estradiol management is non-negotiable, and the dose is fine-tuned alongside testosterone.

The first three months, expectations set honestly

Most men notice energy, mood, and sleep shift in the first two to four weeks. Libido, body composition, and strength typically follow over the first two to three months as levels stabilize and the body responds. Full optimization, where everything is dialed in at the dose your body wants, takes three to six months of careful adjustment.

What we will not do

We will not call your labs normal when you don’t feel normal. We will not start TRT on a single total testosterone reading without the full panel. We will not run TRT without estradiol monitoring, PSA, and intratesticular preservation. And we will not bill insurance for this work, because insurance won’t cover physiologic-dose TRT, the comprehensive panel, or the length of visit it takes. HSA and FSA dollars apply, and we provide superbills for partial reimbursement when your plan allows.

Frequently asked

How do I know if my testosterone is low?

The only definitive way is comprehensive blood work. Symptoms (fatigue, low libido, brain fog, loss of muscle, increased body fat, mood changes) can all point to low testosterone, but they can also be caused by thyroid dysfunction, cortisol imbalance, sleep apnea, or other hormonal issues. That’s why we test the full picture, not just one hormone.

At what age should men consider hormone optimization?

Testosterone begins to decline gradually around age 30, dropping about one to two percent per year. Many men start noticing symptoms in their mid-30s to 40s, though hormone imbalances can occur at any age. If you’re experiencing symptoms, your age matters less than getting tested.

Is testosterone replacement therapy safe?

When prescribed and monitored properly, yes. We monitor your labs regularly, including hematocrit, PSA, estrogen, and liver function. Estrogen management and intratesticular preservation are built into the protocol, minimizing the risks associated with testosterone therapy.

How quickly will I notice results?

Most men begin noticing energy, mood, and sleep shift in the first two to four weeks. Libido, body composition, and strength typically follow over two to three months. Full optimization usually takes three to six months of dose adjustment.

Will I need to be on hormone therapy forever?

Depends on your situation. For some men, hormone optimization is a long-term commitment that provides ongoing benefits. For others, addressing underlying causes like thyroid dysfunction, cortisol imbalance, or sleep apnea can restore natural hormone production. We work with you to determine the right long-term approach.
